Defect Summary

Luxfer canada ltd. (luxfer) is recalling certain unified fine pitch thread adapters, model ep-o-350-8-u5 with part number k0079. The o-ring recess in the gas cylinder adapter may be incompatible with certain valve types.

Safety Consequence

Using an incompatible valve may reduce the o-ring compression, increasing the risk of a hydrogen leak and fire.

Corrective Action

Luxfer will provide the customer with a replacement ufp adaptor, free of charge. Owner notification letters were mailed april 1, 2026. Owners may contact luxfer customer service at 1-403-720-0262.
